Access/Mobile Crisis/Screening Triage Referral (STR)
​​​​GAST services are available to caregivers of consumers who have made contact with Access, Mobile Crisis or STR service when the following criteria are met:
  • The person being cared for in the private home by a non-professional caregiver (usually family member or friend) must be at risk for an imminent psychiatric hospitalization or recently returned home after a psychiatric hospitalization.
  • The person being cared for must be age 60 or above. If the person being cared for is under age 60, he/she must have a diagnosis of demenita or Alzheimer's diseaseto qualify for GAST services.
  • The caregivers (non-professional) should be referred to GAST by Access, Mobile Crisis, STR (Screening, Triage, Referral) or a hospital discharge planner, if there are indications that GAST services might prevent a hospitalization.
  • If the individual or the caregiver is in a current state of crisis, GAST cannot respond.

or

  • The consumer of the mental health provider is psychiatrically hospitalized with a discharge plan to go to an adult care or family care home in a Cardinal Innovations Healthcare Solutions county or in Mecklenburg county.

Some examples of when a mental health provider might make a GAST referral are:

  • Mobile Crisis or other crisis provider is involved in a situation where a psychiatric hospitalization is not currently indicated, but the caregiver needs education to better understand the needs/behaviors of the consumer and GAST services may prevent a future hospitalization.
    • For example, Mobile Crisis was called due to the challenging behaviors of a person with Alzheimer’s disease. The need for inpatient treatment of the person is ruled out by Mobile Crisis, but the caregiver needs help in understanding and coping with the person’s behaviors to help avoid future crisis situations.
  • Mental health provider has a consumer who is in the hospital with a discharge plan to return home to the care of a family member or friend. GAST can provide education/consultation to the caregiver in anticipation of, or after the discharge.
  • Mental health provider’s consumer is currently psychiatrically hospitalized and has a discharge plan that includes living in an adult care or family care home in a Cardinal Innovations Healthcare Solutions county or in Mecklenburg county. GAST can provide education/consultation to the staff of the appropriate adult care or family care home in anticipation of, or after discharge of the consumer to the facility.

GAST services are always FREE. We do not provide direct clinical services, nor do we bill for our services. Our target audience is the person who is caring for someone who has a mental illness.
