Deinstitutionalization (b) (3) Service Array Service Definition allows individuals who are moving from an ICF-IID facility to a setting within the community to receive needed NC Innovations Waiver services without occupying a NC Innovations Waiver slot because the service array is funded by 1915 (b) (3) dollars.

Intended Population

  • This service is available to North Carolina Medicaid participants over 3 years of age who are being discharged from an ICF-IID facility and who meet functional eligibility requirements for the 1915 (c) Waiver (NC Innovations Waiver) program but who are not currently enrolled in the Waiver.
