Personal Care – Innovations

Personal Care Services under North Carolina state plan differs in service definition and provider type from the services offered under the wa​iver. Personal Care services under the waiver include support, supervision and engaging participation with eating, bathing, dressing, personal hygiene and other activities of daily living. Support and engaging the participant describes the flexibility of activities that may encourage the participant to maintain skills gained during habilitation while also providing supervision for independent activities. This service may include preparation of meals, but does not include the cost of the meals themselves.

Intended Population

  • This is a service offered under the NC Innovations Waiver. This service is available to any NC Innovations waiver participant that needs supervision completing skills that have already been acquired or needs physical assistance completing activities of daily living. This service is not available to participants who receive Residential Supports.

Best Practices​


  • Systematic increase of Personal Care hours as In Home Skill Buildling hours decrease when supervision is necessary to maintain skills or when physical assistance is needed to complete activities of daily living

  • Transition from Personal Care to community-based services such as Community Networking and Supported Employment

  • Transition from paid staff to use of equipment or Assistive Technology when appropriate

  • Systematic decrease of Personal Care hours when individual is able to increase amount of time unsupervised ​

  • For individuals under Benchmarks, In-Home Skill Building is subject to the Limits on Sets of Services as specified in the service definition. 

  • For individuals under Support Needs Matrix, In-Home Skill Building is subject to the amount of the individual's Support Needs Category Budget as specified in the service definition. 

  • Providers will be monitored through clinical review activities that focus on utilization patterns and clinical appropriateness of service.
