Home Modifications – Innovations

Home Modifications are physical modifications to a private residence that are necessary to ensure the health, welfare, and safety of the participant or to enhance the participant's level of independence. This service includes an exhaustive list of purchases, installation, maintenance, and as necessary, the repair of home modifications required to ​enable participants to increase, maintain or improve their functional capacity to perform daily life tasks that would not be possible otherwise.

Intended Population

  • This is a service offered under the NC Innovations Waiver for participants receiving provider directed services. This service is available to individuals that have an identified need for a home modification. A physician's signature is required for certifying medical necessity of the requested modification.

  • The service is limited to expenditures of $20,000 during the lifetime of the current NC Innovations Waiver. The NC Innovations Waiver is typically approved for a period of 5 years.
