Community Networking

Community Networking services are intended to provide support for activities that enhance the participant's opportunities to develop meaningful community relationships in integrated community settings. The Community Networking service enables the participant to increase or maintain their capacity for independence and to develop social roles valued by members of the community. This service includes a combination of training, personal assistance, and supports as needed by the individual to participate in identified community activities.  As participants gain skills and increase community connections, the service hours should fade.

Intended Population

  • This is a service offered under the NC Innovations Waiver. This service is available to any NC Innovations waiver participant that needs support to develop meaningful community relationships in integrated community settings.

Management Approach

  • For all individuals, the maximum payment for participant plan year for attendance at classes and conferences does not exceed $1,000.00.

  • For individuals under Benchmarks, Community Networking is subject to the Limits on Sets of Services as specified in the service definition.

  • For individuals under Support Needs Matrix, Community Networking is subject to the amount of the person's Support Needs Category Budget as specified in the service definition.

  • Providers will be monitored through clinical reviews activities that focus on utilization patterns and clinical appropriateness of service.
