As a member of Cardinal Innovations, you can access your health care information and share your information with other organizations or applications that you choose.

The Cardinal Innovations Member Portal is a secure website for:
  • Recent claims made by your provider(s)
  • A directory of providers in our network
  • Ability to grant access for organizations or applications to your information

Access your personal health records easily using a desktop or laptop computer (and for some locations a mobile app).
Login
To log in to the Cardinal Innovations Member Portal, visit portal.cardinalinnovations.org.
If this is the first time logging in to the Member Portal, you will need to register for a new account.

Members
To register for a new account, you will need:
  • An email address
  • Your first and last name
  • Your date of birth
  • A Medicaid ID or CNDS number
  • To create a username and password

Parents, Guardians, or Authorized Representative
If you would like access to someone else’s member information and you are their parent, guardian, or an authorized representative, call the Cardinal Innovations Member Services line at 1-833-580-1965 to receive your ID to register for a new account.

In this portal, you will find claims sent to us by your provider. These claims are for services you have received. You will see how much was paid for this claim and an Explanation of Benefits (EOB).

Your historical claim information is available as far back as January 2016. There may be a delay in when you can see information about more recent services. Certain providers can submit their claims up to 180 days after the service has been given. As soon as the claim is processed, you will have access to view claim details within one (1) business day.

Third-party applications may join our App Gallery using a secure, standards based (HL7 FHIR) API. To be eligible, developers must first register with our App Gallery software partner. After completing the registration and app review process, Cardinal Innovations members will be able to connect their health information from our Member Portal to a third-party application.
