Cardinal Innovations Healthcare has released the 2021 Community Needs Assessment Surveys. Every year we ask members, providers, and stakeholders about their experiences with services so we can better meet the needs of our communities.
We've created two surveys: one for members and one for providers and stakeholders.
Each survey asks about members' experiences with services. Specifically, these surveys examine:
- Possible barriers keeping members from accessing services
- Any service gaps and needs within our communities
- How members feel about their services
The surveys are voluntary and anonymous.
The Purpose of these Surveys
Once we complete these surveys, we will include the information gathered in our 2021 Network Adequacy and Accessibility Analysis (NAAA). The NAAA is an annual report that helps us identify and correct the behavioral health care needs and gaps in our service area.
When and How to Take the Surveys
Our survey opens
Monday, March 1, and closes at the end of the day on
Wednesday, March 31. Members and providers can take the survey online through links we share on our website, our Community newsletter, social media, and InfoSource.
For Members
To receive a survey link through text or email, members can call our Access Line at
1-800-939-5911. They can also call the Access Line to request a paper survey to be mailed to them. Some provider offices throughout our communities will also hand out paper surveys.
Members can
take the English survey here.
Para la versión en español, nuestros miembros pueden

For Providers and Stakeholders
Our Community Operations, System of Care, and Child Program teams will be emailing the survey links to our providers and stakeholders.
